Please take your time to fill out these additional forms as completely and accurately as possible and submit the requested documentation.

• Program Release / Waiver: All participants on ISDSI programs are required to acknowledge and sign the program release / waiver.
• Health Form: To best care for our students we need to know any existing medical conditions or concerns, including allergies and other medical conditions.
• Essential Eligibility Criteria acknowledgement. This documents explains the basic requirements that all students need to agree to before participating in the program.
• Covid vaccination certificate: We require all program participants to be fully vaccinated for COVID with a booster within the last year before travel. This is for your own protection as well as for the protection of the communities we visit. Please send a copy of your vaccine certificate to [email protected].
• Proof of insurance: We require all participants to be covered by travel/health insurance for the duration of their stay. If your group does not have a group plan, please submit confirmation of insurance to [email protected]. Some companies students have used in the past include World Nomads and INext.
